HealthFrame Forms & Reports
HealthFrame provides a wealth of reports to view, summarize, export or print progress charts,
family history, "emergency cards" for your wallet, as well as health summaries to
bring to your provider visits.
Medical History Report
Use the Medical History Summary report to print relevant medical health
information summarized in a convenient format. This report is useful when
preparing for visits with health care providers.
As with most HealthFrame reports, you can configure what information will appear
on the report, leaving out whatever parts you would like.
Emergency Card
Use the Emergency Card to design and
print your own personal emergency information card.
You can configure what information (e.g. which conditions, etc) will appear on your card, and include
your own special instructions or notes.
Family History Summary
Use the Family History Summary report to print a detailed view of the medical conditions of your blood relatives.
This can be critical information to share with your doctor, to help him/her to choose the best medical diagnostic tests to perform on you.
Many diseases, like cancer, hypertension, and heart disease have a strong genetic component.
